NAME OF ARTIST IN RESIDENCE/BIOGRAPHY:
DESCRIPTION OF PROPOSED PROJECT: Include mention of purpose, relevance to existing programs, number and level of students to be affected, involvement of other staff members, methods, materials, and procedures.
COST ESTIMATE: Indicate whether amount requested represents total or partial funding of project and whether there are other possible sources of funding. Break costs into materials/resources, travel, fees, hotel/meals, etc.
TIME FRAME: Account for both planning and implementation phases. Project must begin and end during the 2010-2011 calendar year.
EVALUATION: How will success of project or program be measured? What can be presented or communicated to the Foundation Board?
